House Call Practice in NY

housecalls.jpgDoing house calls seems to be a popular topic these days. Here is a question from an NP in NY…

I  currently am being HEAVILY recruited to be the first NP in a practice of a well established MD doing home visits.  He will let me follow him  for as long as necessary and then start advertising to expand the practice into a business (with his son in law and grandson as principals).. I am not so naive to allow myself to have a practice hinge on NPs doing the work without some real benefit. (part ownership?) .  I am at this moment being offered the possibility of being an employee.  Have I mentioned they REALLY want me?? 

He is offering a generous package (I think) but perhaps he has not made himself familiar with all of my NP issues.  I plan to order one (or more) of Carolyn Buppert’ s books tonight.  How can I get more info?  I would love to have a part ownership at some point. 

Tax issues are also a big problem for me and my husband.  We have 3 kids in college and are both employees with all of our income on W-2s and few deductibles.  Thank you in advance.

    If you are a member of the NYS Nurse Practitioner Association (www.thenpa.org), you can e-mail or call the practice issues consultant with any specific questions regarding ownership. Oh, and I’m the Membership Chaiperson too, so please join if you aren’t already a member :-)

    Good for you! Good Luck in making this effort happen. I can tell you that when I started out 7 years ago, I was roasted over the coals by the doctor faction of the state board of nursing (at that time they pulled the strings on the nurse puppet on the board who did NOT support us as NPs whatsoever) were up in arms about NPs doing such a thing because we would be STEALING doctor’s patients - How dare we even think of doing such a thing? But as their lobbyist (big buffoon of a man) told me in the lobby - I had a great idea for doctors and he was going to have to tell them but that nurses had no right to invade on the doctor’s territory. Please give me a break.

    Anyway, I might have some information that may be useful to you if you will contact me privately I will share what info I gleaned at that time and still use from time to time here in south Louisiana.

    Good luck again and do look at the taxes and pay them out at the end of each quarter so they don’t accumulate.

    Rose,
    I got one reply from you and sent you a long one also but didn’t get a reply so we must be missing the boat somewhere along the way.

    Barbara has suggested that we have a brainstorming call on the NPbusinessowners web site (nominal fee to join but is private so nothing gets out on what is said) because there are several people who have expressed an interest in the same topic.

    So think about that and get back with me. This also applies to Grace and anyone else who may be interested.

    I am presently looking to start a NP House Call business in Washington, DC and beginning to lay the foundation. My one question to you is have you looked at starting your own business without the MD?

    For anyone out there, I have questions about the billing. I plan to be pay and serve, no insurance. However, I am having questions about billing with the codes. Can anyone help me and direct me to somewhere I can get the information.
