I clicked on the link, and it took me to Amazon, but says it is unavailable, and it is not known when it will be available. This sounds like a good resource.

I am starting a business in Pa and need a to submit a practice agreement to the state. Has anyone gone through this process? Did you write your own agreement? Has anyone used the standardized agreement by Bimbela Publications?
