You may want to check the American Medical Directories Association. There are clinical practice guidelines, tool kits, forms and tools, protocols, etc. You'll find the tools here:  http://www.amda.com/tools/index.cfm

Also, you may want to search the AAFP Practice Management journal. They have a "toolbox" with all sorts of encountet forms. http://www.aafp.org/online/en/home/publications/journals/fpm.html

Chances are you will need to find a form you like and add the codes you want. If you do, consider sharing it.
